Benefits Frequently Asked Questions

When am I eligible to sign up?You are eligible to sign up during the first 15 days of employment. If you sign up during the first 15 days, your coverage will begin on the first of the month following your first 30 days of employment. If you have had Volt insurance or Volt COBRA within the last year, your coverage will be effective on the first of the month following your hire date. There is no waiting period and you cannot elect to choose a later effective date. If you do not choose to sign up during the initial enrollment period you must wait until open enrollment.
If you have a qualifying event in your family (marriage, divorce, birth, adoption, etc.), please contact the Benefits Coordinator to find out enrollment qualifications.
How do I know which plan to choose?Please see the benefits comparison sheet for a comparison of the plan options.
What is the Cost?As a benefit to our employees, your weekly deductions are tax-deferred and Volt contributions vary depending upon your choice of health carrier and plan.

What forms do I need to complete?Please check with your Employee Relations Representative or Volt's Benefits Coordinator for details.

***Completed Life Insurance forms must be turned in with the completed medical insurance enrollment forms.

Is it necessary for me to choose a specific PCP (Primary Care Practitioner) at this time?You are NOT required to choose a primary care practitioner anymore. This only applies to the dental enrollment form. If you chose the dental DMO plan it is still necessary to select a provider.
What is the deadline for turning in my enrollment forms?All four enrollment forms (medical, dental, life insurance, and advance medical premium authorization) must be turned in together within 15 days of your start date. Your coverage will then begin on the first of the month following the first 30 days of employment.
Example:
Start date 10/15/2004
Submit date 10/30/2004
Effective date 12/01/2004
*If you do not enroll during this time you must wait until the next open enrollment to do so.
Do returning employees need to have a waiting period before their benefits restart on the next assignment?Employees rehired within 1 year from their previous term date AND who had insurance at the time of termination DO NOT have to satisfy the waiting period. Their benefits effective date will be the first of the month following the rehire date.
How do I know what my effective date will be?Your coverage will be effective on the first of the month, following the first 30 days of employment. However, if you have had Volt insurance or Volt COBRA within the past year, your effective date will be the first of the month following your hire date since there is no waiting period. If this is the case, you can not elect to choose a later effective date.
If I sign up, when will I start seeing deductions?Upon enrollment, four weeks of advance premiums will be deducted over a four-week period. Upon completion of your assignment, coverage will continue through the last day of the completion month. The employee is refunded any premiums that are not needed. Advanced premiums are deducted after-taxes, regular premiums are pre-tax. Remember, you have 15 days from your start date to turn in your enrollment forms, so turning them in as quickly as possible will help to avoid any retro premiums being deducted from your paychecks.
What are advanced premiums, and when will they be deducted from my checks?Advance premiums are taken out to ensure the continuation of your coverage throughout the end of the month in which you, the employee, last worked. They will be deducted for four continuous weeks, beginning 3-4 weeks from the date that you submitted your enrollment forms. The sooner you get your benefits paperwork turned in for processing, the sooner the advanced premiums will start being deducted. This will help avoid having double deductions come out of your paychecks. Any unused advanced premiums will be refunded to you.
When will I receive my health insurance cards?It will take approximately 4-6 weeks from the time you turn in your enrollment forms to receive your United Healthcare insurance cards. You may log onto www.myuhc.com and set up an account with them. Once you are in their system, you will be able to print your temporary ID cards The sooner you turn in your forms during the enrollment period, the sooner you will receive your insurance cards. If you have not received your cards after 6 weeks, but are seeing medical deductions from your paycheck, please don't hesitate to contact the health insurance carrier directly to inquire on the status of your cards.
What if I have a family member who lives out of the service area?Please contact the Benefits Coordinator for additional information about coverage outside of the service area.
